Child Growth & Development
Appointment to monitor your child is meeting growth standards regularly in the first year and yearly there after. This apt is also to be booked if your infant is failing to thrive, or not meeting normal growth standards
Chronic Pain
These appointments are for any patient that is suffering from chronic pain that is being treated by Narcotics.
Complex Care Plan
This apt is done yearly for patients that suffer from multiple disease or disorders that a set plan of action has been discussed. This form is gone over in detail and you receive a copy with your signed acknowledgment of your plan towards improved health.
Drivers Medical
For patients that require yearly medical exams in order to maintain a licence, and for those patients that have specialty licenses that require an exam every couple of years.
First Prenatal Apt
Initial prenatal care after confirmation has taken place. Is done at 10-12 weeks and includes a full head to toe exam, May include pap test and swabs depending on date last completed. Paperwork to be completed and full history taken.
Follow Up Prenatal
Follow up apts done monthly until your prenatal care is assumed by low or high risk maternity.
Form Completion
Any forms that require either an exam and form completion or a review of your file and form completion. - Please be advised there are set fees for form completion based on amount of time or difficulty of completion. This fee must be paid prior to the form being completed or prior to pick up of completed form. - See Uninsured service list of fees for amounts and speak to reception prior to seeing the Physician.
IUD Consult & Insertion
This apt requires an referring Physician and your most recent pap results. If you have not had a pap then let the staff know upon booking so they may adjust the time frame to include the pap exam during the consult. Once a decision has been made to proceed to IUD insertion you will receive all information about preparation for the procedure. You will receive a prescription prior to insertion you must pick up your IUD at the pharmacy and bring it to your apt with you.
IUD Removal
IUD's are a form of birth control that is good for up to five years at which point you will need to book for removal and either a reinsertion of a new IUD or discuss other forms of birth control.
Initial Consult
A history appointment made for the first time you meet a Physician who is accepting new patients and you want to continue seeing as your Family Doctor.
Mental Health
For any patient that wishes to discuss a new or follow up concern with stress, depression , ADD /ADHD, anxiety, or Schizophrenia.
Minor Surgery
For removal of any lesion that a Physician has agreed can be removed in office and does not require specialist intervention. Moles, Lipomas, Skin tags that are in an area of friction. This is covered by AHC unless the skin tags are only being removed for cosmetic purposes. See uninsured fee guide.
Pap Test
This is for patient's that have had abnormal pap's in the past and medically require repeat screening outside of the regular testing schedule
Post Natal
Typically done at 6 weeks post partum to ensure healing and comfort.
Pregnancy Confirmation
Appointment to confirm a pregnancy in office.
Prescription Refills
Apt to refill medications only.
Psychological Services
Due to Covid-19, sessions can be via confidential video sessions or walking in an outdoor space. However, in-person sessions are also available within the Oakridge Medical Clinic, following Covid-19 precautions. Free 15-minute consultation., Fees are standard as per the Psychologists' Association of Alberta: $200 per 50-minute session, Most extended health insurance plans cover all or a portion of the costs, I work directly with the Alberta Medical Association, Services are not covered by Alberta Health Care.
Recall
our office has called and left a message to inform you that the Physician would like to discuss a concern with your Lab or Diagnostic results. They will be indicated if urgent when contacted, if not stated urgent you may assume you can book within 2 weeks.
Results
If you would like to review your lab or diagnostic test results, even though there was no call to inform you of a concern
Travel Consults
Appointments for patients that will be traveling outside of Canada and require information and medication or injections prior to travel.This is not covered by AHC and is an uninsured service - see fee guide.
WCB Follow Up
Continued follow up appointments to monitor the healing process of the injury . Forms are completed each time.
WCB Initial
The first time you are seeing a Physician at our location for an injury aquired at work . Forms are completed . If you were seen in Emergency or another clinic initially you need to bring your WCB claim number and site your were seen at originally with you.
